Notice of Privacy Practices

Aligned Medical Massage is committed to the confidentiality, integrity, and security of patient Protected Health Information (PHI) in compliance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable Oregon state laws.

Permitted Uses and Disclosures

We may use and disclose your PHI for treatment, payment, and healthcare operations, adhering to the minimum-necessary standard. This includes sharing information with your other healthcare providers to coordinate care, and with insurance companies — including PacificSource / OHP — for billing and claim management. Other uses and disclosures not described in this notice will be made only with your written authorization, which you may revoke at any time.

Your Rights Regarding Your PHI

You have the right to: request restrictions on certain uses and disclosures of your PHI; receive confidential communications; inspect and copy your health records; request amendments to your health information; receive an accounting of disclosures; obtain a paper copy of this notice; and file a complaint if you believe your privacy rights have been violated.

Breach Notification

You will be notified in the event of a breach of unsecured PHI, as required by HIPAA.

Minor Patients

We respect the privacy of minor patients. Parents or legal guardians have access to their minor child's PHI, except where minors can legally consent to their own treatment under Oregon state law.

Language Assistance

We provide language assistance services at no cost to PacificSource members with limited English proficiency or those who are deaf/Deaf or hard of hearing, including oral interpretation and written translation of vital documents.

Text Messaging

Standard text messaging is not secure under HIPAA regulations. If you choose to communicate with us by text, messages may include limited PHI and may be visible to others if your device is unattended or compromised.

Complaints & Contact

You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.

Changes to This Notice

We reserve the right to change this notice and will provide a revised notice as required by law. The current version will always be posted on this page and available at our office.
